This section gives you information and access to the official MOD Forms required to register for your various resettlement activities, from initial registration to work placements and undertaking CTP training courses.
They can be downloaded and printed off but remember to get the required signatures (as stated on the form) from your unit and Service Resettlement Adviser before submitting the completed version to the CTP.
MoD Form 1173
Application for Resettlement Services. This is the initial form that is required to be completed in order to register for your resettlement provision.
MoD Form 363 (with Guidance Notes)
Authority to Attend Resettlement Activities (such as CTW, other workshops, briefings, etc) but not for training courses (use Form 363T below).
MoD Form 363T (with Guidance Notes)
Authority to attend Resettlement Training Activities. This form is only to be used for Resettlement Training purposes. All other Resettlement Activities should be applied for using Form 363 (above).
MoD Form 1746
Form of Agreement for Civilian Work Attachment or External Training for a Member of the Armed Forces.
MoD Form 1748
Attendance at CWA/CTA Confirmation and Claim Form. This form is to be used on completion of a Civilian Work Attachment or undertaking External Training in order to claim refund of fees under the IRTC Scheme.

Application for Standard Learning Credit (SLC) funding.

Early Service Leavers - Record of Resettlement Inteviews.
MoD Form 1711
Record of Resettlement Provision.
MoD Form 2225
Resettlement Complaints Form. If you are a Service leaver who has a complaint about the resettlement provision received, you should speak with your Service Resettlement Advisor who will raise this form for you as the start of the complaints procedure.
MoD Form 2245
Application for Refund of Resettlement Allowances and Entitlements. This form should be used by Service leavers wishing to reclaim GRT, Travel Warrants, IRTC Grant and/or Personal Contribution lost due to unforseen circumstances.

This questionnaire is for completion by ex-Service personnel at 6 months post discharge.
Time of Discharge Questionnaire
This questionnaire is for completion by Full Resettlement Programme Service leavers at the time of discharge.
ESP Service leavers at 6 months
This questionnaire is for completion by Employment Support service personnel at 6 months post discharge.
