MOD Forms
IMPORTANT INFORMATION - Please Read: How your resettlement will be administrated will depend on when you became eligible to commence resettlement. Many of the MOD Forms below have been integrated into JPA and can be accessed via your self service menus. This method applies only to those Service Personnel who became eligible to commence their resettlement on, or after 15 March 2010. If in doubt, contact your Service Resettlement Adviser.
MoD Form 363 (with Guidance Notes) (Use JPA if eligible on, or after 15 March 2010)
Authority to Attend Resettlement Activities (such as CTW, other workshops, briefings, etc) but not for training courses (use Form 363T below).
MoD Form 363T (with Guidance Notes) (Use JPA if eligible on, or after 15 March 2010)
Authority to attend Resettlement Training Activities. This form is only to be used for Resettlement Training purposes. All other Resettlement Activities should be applied for using Form 363 (above).
MoD Form 1711 (Use JPA if eligible on, or after 15 March 2010)
Record of Resettlement Provision.
MoD Forms 1173A (Use JPA if eligible on, or after 15 March 2010)
Early Service Leavers - Record of Resettlement Inteviews.
MoD Form 1173 (Use JPA if eligible on, or after 15 March 2010)
Application for Resettlement Services. This is the initial form that is required to be completed in order to register for your resettlement provision.
MoD Form 1746
Form of Agreement for Civilian Work Attachment or External Training for a Member of the Armed Forces.
MoD Form 1748
Attendance at CWA/CTA Confirmation and Claim Form. This form is to be used on completion of a Civilian Work Attachment or undertaking External Training in order to claim refund of fees under the IRTC Scheme.
MoD Form 1950
Application for Standard Learning Credit (SLC) funding.
MoD Form 2225
Resettlement Complaints Form. If you are a Service leaver who has a complaint about the resettlement provision received, you should speak with your Service Resettlement Advisor who will raise this form for you as the start of the complaints procedure.
MoD Form 2245
Application for Refund of Resettlement Allowances and Entitlements. This form should be used by Service leavers wishing to reclaim GRT, Travel Warrants, IRTC Grant and/or Personal Contribution lost due to unforseen circumstances.
Questionnaires
6 month post-discharge questionnaire
This questionnaire is for completion by ex-Service personnel at 6 months post discharge.
Time of Discharge Questionnaire
This questionnaire is for completion by Full Resettlement Programme Service leavers at the time of discharge.
ESP Service leavers at 6 months
This questionnaire is for completion by Employment Support service personnel at 6 months post discharge.
